There is a way around that - the breast massage skill from the gladiator will make your player masturbate even if they weren't planning on it when they started playing with their tits.

A bit more on ballgags
There will be some normal ballgags in the game as well as the cock pacifier. These will come in varying sizes, larger ones being worse.
