There will be some normal ballgags in the game as well as the cock pacifier. These will come in varying sizes, larger ones being worse. The larger the ballgag, the more it makes your jaws ache, which of course makes it more difficult to concentrate on everything else that is happening. This is reflected in the game by your intelligence taking a significant hit whilst wearing large ballgags.
I'm still working on the varying ways that these ballgags might actually find their way onto your person! The witch will definitely be one way, and there'll be at least one clothing and bondage applying trap in the hotel level.
could always add them as a consequence to frequent talking/begging against some monsters, like how guards will add cuffs if you have the cursed latex plug panties on
